Access to the Clearance Form for Separating Faculty and Staff is available online. 

Bring your form to the library services desk at any Perimeter campus library, or Atlanta campus library on the first floor of Library North. 

Your account must be clear of all fines and/or fees and all library items/materials must be returned before we can sign off on the form.
